Please see attached for how to enroll in your Health Savings Account, if you've enrolled in either the Cigna Medical Base Plan or Buy-Up Plan.

IMPORTANT: As a HSA participant, you pay a monthly fee of $2.25 charged to your account if your account balance is under $3,000.
